Care One Home Health Services, Inc. is a minority, woman, and Registered Nurse owned and operated healthcare agency. Established in 2005, Care One has grown to offer the following medical and non-medical services: Home Health, Private Duty Nursing, Home Services, and Agency Staffing.
Our Services
Home Health
Home health services will help you transition back to home upon discharge from your most recent inpatient stay in the safest way possible. We complete our Initial Assessment within 48 hours of your discharge. Additionally, your Primary Physician may refer you to home health services if you are homebound with medical necessity. These intermittent visits from our clinical staff will help you heal in the comfort of your home. We provide the following clinicians:

Private Duty Nursing
Private duty nursing is a solution to prolonged skilled nursing needs in the home setting. Our clinicians are matched with clients according to their condition for the best patient-centered care and outcomes. Clinical experience of our staff include the following:

G-Tube Feeding / Care
Home Services
Home services is the non-medical services option for clients looking for assistance with Activities of Daily Living (ADLs) - personal care, household tasks, medication reminders, and meal preparation. Our Initial On-Site Assessment is conducted by a Registered Nurse, and they will work with you to develop a Client Care Plan tailored to your individual needs. We provide live-in, 24-hour, and come-and-go caregivers.
Agency Staffing
Agency staffing is for facilities who wish to contact with our agency as a solution to staffing shortages. We provide Registered Nurses (RNs), Licensed Practical Nurses (LPNs), Certified Nurse Assistants (CNAs), and Sitters to a number of skilled nursing and assisted living facilities in the Chicagoland area.
Home Health / Private Duty Nursing: To start care, consult with your referring physician. They will be able to coordinate care with us.

Home Health Services
Insurance Coverage
In most cases, home health is an insurance-covered service with no to minimal coinsurance payment. Prior to the start of services, we have the ability to run an eligibility check for insurance coverage. For home health services, please visit your Primary Care Provider (PCP), and if you qualify for services, they may send us a referral to start care.
We accept the following insurance plans:
Medicare, Medicare Advantage PPO Plans, Veteran’s Administration, Tricare
Private Duty Nursing
Insurance Coverage or Private Pay
If your commercial insurance plan covers Private Duty Nursing, we will work closely with your insurer to obtain authorization and payment for services rendered. Our team will ensure that your insurer receives all required medical records. We also offer a private pay option for clients who do not have commercial insurance coverage.

Private Pay
Non-medical home services is typically an out of pocket expense for our clients. Our competitive rates span across our variety of services. We have daily live-in rates as well as hourly rates for our caregivers. We require a minimum of 4 hours per day for come-and-go services.
