Founded in 2005, Care One was built on a strong clinical foundation and a deep understanding of patient-centered care.
Over the years, we have grown to provide a comprehensive range of medical and non-medical services tailored to meet the unique needs of individuals and families. Our services include Home Health, Private Duty Nursing, and Home Services, all designed to promote independence, safety, and improved quality of life.

Home Health
Home health services are designed to support a safe and smooth transition back to your home following discharge from your most recent inpatient stay. To ensure continuity of care, our team completes an Initial Assessment within 48 hours of discharge.
Your Primary Care Physician may also refer you for home health services if you are homebound and have a medical necessity. Through scheduled, intermittent visits, our skilled clinical staff provides the care and oversight needed to promote recovery—allowing you to heal safely and comfortably at home.

Our Services
Private Duty Nursing
Private duty nursing provides individualized, extended skilled nursing care in the comfort of your home. This service is ideal for clients who require ongoing clinical support beyond short-term or intermittent care.
Our clinicians are carefully matched to each client based on their medical condition, care needs, and level of acuity to ensure the highest quality, patient-centered care and optimal outcomes. Our nursing team brings extensive clinical expertise, including experience in the following areas: Ventilator & Tracheostomy Care Oxygen Management IV / Fluids InfusionG-Tube Feeding / Care
Home Services
Home Services provide non-medical support for individuals who need assistance with Activities of Daily Living (ADLs). These services include personal care, help with household tasks, medication reminders, and meal preparation—allowing clients to maintain independence and comfort at home.
Each client begins with an Initial On-Site Assessment conducted by a Registered Nurse, who works closely with you and your family to develop a personalized Client Care Plan based on your specific needs and preferences.
To accommodate varying levels of support, we offer flexible care options, including live-in care, 24-hour care, and come-and-go caregiver services.
Home Health / Private Duty Nursing: To start care, consult with your referring physician. They will be able to coordinate care with us.

Home Health Services
In most cases, home health services are covered by insurance with little to no out-of-pocket cost. Prior to initiating care, our team can verify your insurance benefits and eligibility to help you understand your coverage.
To begin home health services, please schedule a visit with your Primary Care Provider (PCP). If you meet the criteria for home health care, your PCP can submit a referral to Care One Home Health Services to initiate care promptly.We accept the following insurance plans: Medicare, Select Medicare Advantage PPO Plans, Veteran’s Administration
Private Duty Nursing
If your commercial insurance plan includes coverage for Private Duty Nursing, our team will work directly with your insurer to obtain the necessary authorizations and coordinate payment for services provided. We handle the submission of all required medical documentation to help ensure a smooth approval process.
For clients who do not have commercial insurance coverage, we also offer flexible private pay options to ensure access to the care you need.
Home Services
Non-medical Home Services are typically an out-of-pocket expense; however, some services may be covered by commercial long-term care insurance plans. Our team is happy to assist with benefit verification when applicable.
We offer competitive, transparent pricing across our range of home services, with flexible care options to meet your needs. Rates include daily live-in care as well as hourly caregiver services. For come-and-go care, a minimum of four (4) hours per day is required.
